Those NOINDEX and NOIMAGEINDEX directives are honored by google and by most other search engines. One possible solution would be for Flickr to pass the NOINDEX directive in the HTTP header (or in the page "head" section) for the Moderate photo pages, and to add the NOIMAGEINDEX directive in the HTTP header of the served Moderate images. Note: I made this image Moderate, and then to make sure I refreshed its secrets, so they became different from the secrets of the image when it was "safe". Here is an example (one of my images set to Moderate on Flickr): pops here in Google search results: So google is doing just that, and it fetched the image and is displaying it (in the largest size available from the photo page) in the google image search results.
